Dear Patient,

It is with mixed emotions that I write this letter to inform you that I am transferring to our clinic in West Enid effective April 2nd. I know that with this move will come loss of relationships that I have cultivated over the past few years. I have truly enjoyed getting to know you and your family and being a part of your lives. This decision did not come lightly, but at this time I feel it is best for me along with GSP Health.

I want to make this transition as smooth as possible for you. I will be available to see you in Cherokee up until April 2nd. After this, if you are willing to make the drive, I would be happy to see you at our Enid West office located at 5320 W Chestnut Ave, Enid, OK. I am also available via telehealth. However, I know this is not practical for everyone. There are several other great providers at our Cherokee clinic who would be happy to see you and attend to your health care needs. Our office will be reaching out to reschedule you if you have an appointment with me after April 2nd. At that time, please let reception know who you would like to schedule your follow up appointment with.

Thank you for allowing me to be a part of you and your family’s lives. I have sincerely enjoyed serving you. If there is anything that needs to be addressed with me prior to my departure from the Cherokee office, I encourage you to schedule prior to April 2nd. You can call the clinic at 580-596-2800.
