Requests will be processed as soon as possible but no later than 30 days from receipt.
Patients
You have free access to view, print, and download your records via your Patient Portal, 24/7, 365 days a year at the link below.
For shot records, you also have free access by OSIIS (Oklahoma State Immunization Information System) to view, print, and download your shot records via their own Patient Portal, 24/7, 365 days a year.
You may submit a request to obtain physical/digital copies of your records by completing the Patient Release for Medical Information form. Routine requests will be processed as soon as possible but no later than 30 days from receipt.
