Dear Patient,

I write this letter with mixed emotions to inform you that I will be leaving GSP Health. My last day seeing patients will be March 8th, 2023. I am thankful that you chose me to provide for your healthcare needs over these past 3 years. I have enjoyed getting to know each one of you and your families.

I will continue to see patients until my last day. I would love to transition your care to one of the other great providers at GSP Health. They will be more than happy to continue providing you with the high-quality healthcare you have come to know and expect from GSP Health. 

If you have an appointment scheduled after March 8th, please call the office at 580-395-3200 to reschedule with one of the available providers. If they do not hear from you, they will attempt to contact you to reschedule your appointment. Again, thank you for allowing me to care for you these last 3 years.
